Understanding Prediabetes and Diabetes

Prediabetes is a critical health condition where blood sugar levels are higher than normal but not yet high enough to be classified as type 2 diabetes. With approximately 88 million American adults affected, prediabetes significantly increases the risk of developing type 2 diabetes, heart disease, and stroke. Alarmingly, over 84% of those with prediabetes are unaware they have it.

Diabetes Self-Management Education and Support

At RAMC, our Physicians Group offers a certified Diabetes Self-Management Education and Support (DSMES) program recognized by the American Diabetes Association (ADA). Our Diabetes Educators, who are skilled healthcare professionals, provide evidence-based DSMES services designed to enhance your diabetes knowledge, improve self-care behaviors, and boost your quality of life. These services are tailored to manage all types of diabetes, including type 1, type 2, gestational, and prediabetes.

Key DSMES Services Include:

Medication
adherence

Monitoring blood
glucose levels

Healthy
eating habits

Physical
Activity

Coping and problem-solving skills

Reducing risks of diabetes complications

The ADA recommends DSMES services at four critical points: at diagnosis, during annual assessments, when new complications arise, and during transitions in care. Most DSMES services are covered by Medicare and commercial insurance.

Prediabetes and Diabetes Prevention

Our CDC-recognized Health4U Diabetes Prevention Program is designed to help you make sustainable lifestyle changes to lower your risk of developing type 2 diabetes. By participating in this program, you could reduce your diabetes risk by up to 58% (or 71% for those over 60).

Program Highlights:

  • Working with a trained coach to implement lasting lifestyle changes
  • Learning to eat healthily and incorporate more physical activity
  • Managing stress, staying motivated, and overcoming obstacles
  • Gaining support from peers with similar goals and challenges

Please contact Sandra Jewell or more information or to register at [email protected] or call 608-768-6243 and visit Health4U for more information.
